Team and Player Entry Registration
Team and player registration will be done at the field at least 1 hour prior to your first game.
Player Registration Information Instructions
The instructions below apply to the production of Game Sheets and Bench Staff Sheets. A Game Sheet will serve as your team's Official Icebreaker Tournament Player Roster.
- Please download the Game Sheet file to your computer and open the PDF.
- Once completed please print the file. (if you cannot complete the form digitally, please print the form and fill it out by hand and make the required number of copies)
- Festival Teams – Print 6 copies of the game sheet.
- Tournament Teams – Print 10 copies of the game sheet.
- Please download and complete one (1) copy of the Bench Staff List and submit it when you register your team.
- A Travel Permit is required by all out-of-district teams. If your players' books contain a district stamp other from than the Eastern Ontario District Soccer Association (EODSA), you require a Travel Permit - even if you play in the Eastern Ontario Soccer League (EOSL). Please email us your travel permit once it is approved.
- Quebec teams: Please remember that you need two (2) stamps of approval on your Travel Permits (one from your district and one from the FSQ).
Registration will take place at the field a minimum of 1 hour prior to your first game. There will be a registration tent at each venue.
All players must be present and receive a hand stamp before they can play in the festival or tournament.
U-9 to U-12 teams
You will need to submit the following when you register:
- A copy of your Official Ontario Soccer Team Roster or an Official Quebec Soccer Federation Team roster or a typed list of your team's Registered Players on your Soccer Club's letterhead signed by your club verifying that all players are registered with your provincial association.
- 6 Icebreaker game sheets
- 1 team bench list
- Travel permit if applicable
- All of your players
Every player must be present at registration and will receive a hand stamp. If a player is not available when your team is registered that player must register with the tournament before playing.
U-13 and older teams
You will need to submit the following when you register:
- A copy of your Official Ontario Soccer Team Roster or an Official Quebec Soccer Federation Team roster.
- 10 Icebreaker game sheets
- 1 team bench list
- Travel permit if applicable
- Player cards (actual cards or digital cards
- All of your players
A card check will be performed during team registration. Every player must be present at registration and will receive a hand stamp. If a player is not available when your team is registered that player must register with the tournament before playing.
